Who is SteppingStone?
SteppingStone is the largest provider of Adult Day Health Care (ADHC) in San Francisco. Founded in 1983, our primary objective is to help seniors and disabled adults maintain their health and quality of life, while continuing to live in their homes. By being active and receiving care in a supervised environment, our clients have fewer accidents and hospital stays, thus improving dramatically their quality of life.
SteppingStone (formerly North & South of Market Adult Day Health), is a 501 (c)(3) non-profit organization that operates four Adult Day Health Care Centers that serve residents of San Francisco.
We are licensed by the California Department of Health Services and certified as a Medi-Cal provider by the California Department of Aging. Since 1983, we have worked tirelessly to care for seniors, so they can remain in their homes by providing a cost-effective and comprehensive set of services in one location.
We serve over 300 clients a year in our four Centers that each provide nursing and personal care; medical and social work services; nutritious meals; medication management; physical, occupational, and speech therapies; as well as social and recreational activities. We also offer round trip transportation to and from our Centers via our wheelchair accessible vans
Our unique program model is a community-based alternative that helps prevent unnecessary institutional placement and is designed to allow individuals to age in place with their families and community in a safe, caring, and stimulating environment.
SteppingStone welcomes LGBTQ individuals and celebrates their resilience!
We recognize that these communities have particular needs and concerns,but have historically faced barriers to accessing affirming care.
Funded by a grant from SF's Department of Aging & Adult Services (DAAS), we aim to eliminate disparities by providing trauma-informed, culturally-competent treatment in a safe and non-judgmental environment. Our staff receive ongoing training and support in order to address the evolving needs of LGBTQ communities. We are proud to partner with Openhouse, an organization renowned for their vital work connecting LGBTQ elders with housing and community supports. Together, we are creating a unique LGBTQ-focused adult day health program – the first of its kind on the West Coast!
Who We Serve:
Our program is proud to serve Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ) individuals and their families (of origin and chosen). SteppingStone provides wraparound services and social engagement opportunities for elders & adults with disabilities. We employ a multidisciplinary team of professionals who consult with each participant to develop an individualized plan of care and engagement.
We accept Medi-Cal managed care plans (San Francisco Health Plan and Anthem Blue Cross) and Veteran Affairs insurance, as well as private pay, and can assist eligible participants with enrollment—the majority of our participants do not incur out-of-pocket expenses. Opportunities for Engagement:
Our growing cohort partakes in regular activities scheduling, such as art classes and bingo, along with specialized activity offerings such as Rainbow Cinema Club and LGBTQ-focused support groups. We provide clinical accompaniment and advocacy to support our LGBTQ participants in establishing care and connecting to supplemental services. We’ll soon be launching regularly-scheduled field trips to connect our cohort with Openhouse’s expansive community events.
