SteppingStone Adult Day Health is San Francisco's largest provider of Adult Day Health Care (ADHC). On-Call Occupational Therapist 

Job Summary

Under the supervision of the ADHC Program Director, the on-call Occupational Therapist is part of a Multi-Disciplinary team of health care providers serving an elder and disabled adult population.

Duties and Responsibilities:

  • Provide occupational therapy assessments, treatment plan, and therapy interventions and routines for frail elders and disabled adults in a ADHC program.

  • Oversee work of Rehabilitation Aide in administrating treatment routines.

  • Document treatment plan and interventions in client database.

Qualifications:

  • Graduate of an occupational therapy curriculum accredited jointly by the Council of Medical Education of the American Medical Association and the American Occupational Therapy Association (ACT) and registered by ACT.

  • Experience working with an elder population and with a multidisciplinary team preferred.

  • Strong oral and written communication skills in English.

  • Computer literacy for documentation and reports.

  • Effective organizational skills.

  • Interest and ability to work in a diverse population of staff and clients.

Working Environment

Must be able to safely perform all physical and manual movements needed for job functions. Must be able to carry out job instructions promptly and willingly, and to follow established procedures accurately.  Must be flexible, positive and cooperative in solving problems and working with other staff in constantly changing and sometimes-stressful circumstances.

Physical Demands

The Occupational Therapist must be able to move around the facility as well as sit at a workstation.  Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. Occurs only occasionally.

Physical Therapist

Job Summary

Under the supervision of the ADHC Program Director, the Physical Therapist assesses the functional capabilities of the participants; provides and supervises individual treatment plans; acts as a consultant to the Program Director; and is a member of the Multi-Disciplinary team.

Duties and Responsibilities:

  • Evaluate participants' physical condition and functional status. Establish goals and set up treatment plans to administer physical therapy techniques as prescribed by the referring physician.

  • Develop and implement the therapy maintenance program and supervises work of Rehabilitation Aides.

  • Provide physical therapy treatment; instruct and demonstrate to families and others the physician therapy and rehabilitation procedures indicated by the participant's assessment.

  • Provide general instruction to principles of physical therapy to staff as needed. Teach classes and concurrent treatment.

  • Record in participant's chart the services given, maintain progress notes, and schedule participant for further treatment.

  • Work in close cooperation with staff and participate as a member of the multidisciplinary team.

  • Meet all administrative requirements such as attending meetings, record keeping, etc.

  • Reevaluate the needs for physical therapy for all participants of the ADHC.

  • Must be able to safely perform all physical and manual movements needed for job functions including but not limited to: effectively and safely lifting a 25-pound weight; transferring clients; assisting with personal care, meal service and special events; assisting participants with personal care; escorting clients home; and assisting with loading and unloading of client vans.

  • Other duties as assigned, including training new therapists and rehab aides, and providing in-services to staff.

Qualifications:

  • A Bachelor's Degree in physical therapy.

  • Licensed by the Physical Therapy Examining committee of the California Medical Board.

  • Hold current CPR and First Aide certification.

  • Two years of experience working with the elderly or chronically ill.

  • Experience working in a multidisciplinary team is preferred.

On-Call Behavioral Health Specialist

Job Summary

We are currently looking for the on-call Behavioral Health Specialist to oversee the mental, emotional, and behavioral health needs of a formerly homeless elderly population living in supportive housing (Direct Access to Housing – DAH) and attending Adult Day Health Care (ADHC) in San Francisco.

Duties and Responsibilities

  • Provide the ADHC center staff support for addressing mental, emotional and behavioral health needs of intakes and initial assessments and ongoing assessments for all DAH residents

  • Provide counseling, support and intervention to DAH residents and ADHC clients as needed in areas of behavioral management, caregiver stress, coping strategies, case management and linkage with community resources and other issues as needed

  • Develop and monitor an individual plan of care for each DAH resident with measurable goals

  • Facilitate weekly support and/or education groups that address clients’ psycho-social and educational needs

  • Meet with ADHC social worker and DAH housing social worker on a regular basis to discuss and document client case review

Qualifications

  • Licensed as clinical social worker by the California Board of Behavioral Science or licensed as MFT

  • Knowledgeable of the physical, psychological, emotional, and social needs of the elderly

  • Experience in the provision of counseling, behavioral health therapies, support groups, and assessments preferred

  • Experience in adult day support services is desirable.

Hours & Compensation: 

This is an on-call position, Monday - Friday. The hourly rate for the on-call Behavioral Health Specialist is $23.75/hour.

On-call Licensed Vocational Nurse

Job Summary

Under the supervision of the Registered Nurse, the LVN is responsible for carrying out specific direct patient care duties under the direction of the RN and for helping with personal care, meals, treatments and therapy. Must attend all required staff and training meetings. The hours for this position are 7 hours/day, 5 days/week. 

Duties and Responsibilities

  • Direct patient care:

    • Prepare and administer medications (oral and hypodermic), tuberculin skin tests, and random blood sugar tests, and insulin shots as needed.

    • Provide dressing changes

    • Assist RN with six month and quarterly assessments

    • Measure vital signs; administer neurological checks

    • Provide basic emergency care; CPR, First Aid

    • Assess client for pain

    • Administers nursing skin and nail care to clients as needed

    • Oversee client personal care, and provide assistance as needed

    • Advise meal changes if needed

    • Weighs clients and records information

    • Provide catheter care; sterile irrigations assistance, as needed

  • Daily charting of medications, RBS; progress notes as needed on nursing treatments; flow sheet filing; assist RN with quarterly assessments

  • Assists with lunch set up and feeding

  • If bilingual, provides translation services

  • Attends required staff and training meetings

  • Assists in clean-up of nursing areas

  • Cooperates with all staff as needed for meals service, personal care, special event and other tasks as requested by supervisor or program director. Other duties as assigned

  • May work in other NSM-ADH programs

Qualifications

Must be licensed as a vocational nurse with a current California LVN license. Two years of experience and familiarity with the needs of the frail is desirable. Current CPR and First Aide certificates are required. Must be able to communicate readily and effectively in English, orally and in writing.

Must be able to safely perform all physical and manual movements needed for job functions, which includes transfer of clients, assisting with personal care, meals service, special events, cleaning in rest rooms, escorting client’s home, helping with loading and unloading of client vans, and other needed tasks. Must be flexible, positive and cooperative in solving problems and working with other staff in constantly changing and sometimes stressful circumstances.

  • Ability to lift twenty-five (25) pounds. Moving, lifting or transferring of patients may involve lifting of up to 100 pounds.

On-call Program Aide

The On-call Program Aide is available for fill-in staffing at any of SteppingStone’s four ADHC centers. The On-call Program Aide will provide personal care, participant escort, gym assistance, and activity leadership, and will assist with class instruction, meal/snack service, cleaning, laundry, and other duties as needed. The On-call Program Aide may also be asked to work during program special events. The position reports to the Program Director.

Responsibilities:

  • Room set-up for daily programs; maintains neat and orderly program rooms and restrooms.

  • If bilingual, provide interpretation both at work and in participants’ homes, as needed.

  • Assists participants to and from their buses, cars or apartments if close by; oversees and assists with safety protocols in loading zone.

  • Assists participants with personal care, personal hygiene and grooming.

  • Organizes and distributes lunches, snacks and clean-up, and assists participants with meals, as needed.

  • Helps lead teachers and staff with daily activities and special events/parties, maintains supply log.

  • If assists in gym, must complete daily flow sheets and documentation.

  • Socializes with participants and helps them initiate/participate, monitoring behavior.

  • May open and close premises and make reminder phone calls to participants in the mornings for their van pick up.

  • Other duties as assigned.

Qualifications:

  • A high school diploma and a demonstrable aptitude in working with people with physical and mental disabilities.

  • Language skills desired: Cantonese, Korean, Vietnamese, Tagalog, Mandarin, or Russian.

  • Responsible, flexible, and a team-player.

  • Experience working with frail and elderly adults, people with disabilities, or people with dementia is preferred.

  • Effective written and verbal English communication skills.

  • Ability to safely lift 25 pounds, squat/stand easily, and transfer clients during personal care needs, cleaning restrooms, and escorting clients home.

Hours & Compensation: This is an on-call position, Monday – Friday, 8:30am-2:30pm. The position will work at each of the organization’s four centers, as needed. The hourly rate for the On-call Program Aide is $14.00/hour.

On-call Registered Nurse (RN)

SteppingStone seeks a Registered Nurse. This is an on-call position to be shared among SteppingStone’s four (4) San Francisco ADHC centers.

  • Current CA Registered Nurse certificate required

On-call Social Worker

The On-call Social Worker is available for fill in staffing at any of SteppingStone’s four ADHC centers to oversee direct social work services to the ADHC participants.  The position is supervised by the center’s program director and may provide social work support as relevant to ADHC participants’ individual care plans.  They are available for mental health crisis intervention and liaison with family and social service agencies if needed.  The On-call Social Worker will document services provided in participant’s individual records. 

Qualifications:

  • Masters in Social Work by accredited school of social work or person licensed as a clinical social worker by the California Board of Behavioral Science Examiners

  • Experience with disabled adults and frail elders in a multi-ethnic setting is preferred

  • Ability to communicate readily and effectively in English, orally and in writing

On-call Social Work Assistant
