The ECM team works with participants, their families, and healthcare providers to ensure coordinated and effective care, resulting in improved health outcomes and enhanced quality of life. Every ECM participant will have a Lead Care Manager assigned to support them, accompanied by community health workers, nurses, and other care providers. This team will provide:

• Personalized Care Plans: Each participant has a comprehensive assessment and receives an individualized care plan tailored to their specific needs and goals.
• Care Coordination: ECM staff serve as central point of contact for participants, their families, and healthcare providers. Our care managers coordinate services to ensure that all aspects of care are addressed.
• Regular Monitoring and Support: ECM staff monitors clients’ health status, tracks progress, and addresses emerging issues promptly to ensure they receive the necessary support to manage their health.
• Resource Navigation: ECM staff provides assistance in accessing community resources, coordinating appointments, arranging transportation, and connecting participants with appropriate services.
• Advocacy and Education: ECM care managers advocate for participants’ rights and help them make informed decisions about their care. We also educate clients on self-care and navigating the healthcare system so that they can effectively advocate for themselves.
• Continuity of Care: We work to confirm smooth transitions between healthcare settings for clients through close collaboration with hospitals, rehabilitation centers, and other care providers.
