Enhanced Care Management
Providing Additional Support to People that Need It
The Enhanced Care Management (ECM) program provides comprehensive and personalized support for people who require additional assistance in managing their health and accessing necessary resources. The program primarily works with Older Adults and Adults with Disabilities. ECM is available to those that attend SteppingStone’s Day Health Centers as well as members of the public, however participants must be a San Francisco Health Plan (SFHP) Medi-Cal recipients.
The ECM team works with participants, their families, and healthcare providers to ensure coordinated and effective care. Every ECM participant has a Lead Care Manager assigned to support them, along with community health workers and other care providers. This care team provides:
Personalized Care Plans: Each participant has a comprehensive assessment, and an individualized care plan tailored to their specific needs and goals.
Care Coordination: ECM care managers coordinate services to ensure that all aspects of care are addressed including serving as central point of contact for participants, their families, and healthcare providers.
Regular Monitoring and Support: ECM staff monitors clients’ health status, tracks progress and addresses emerging issues to ensure they promptly receive necessary support.
Resource Navigation: ECM staff provides assistance in accessing community resources, coordinating appointments, arranging transportation, and connecting participants with appropriate services. If needed ECM staff will accompany clients to meetings with other health providers and support agencies.
Advocacy and Education: ECM care managers advocate for participants’ rights and help them make informed decisions about their care. We also educate clients on self-care and navigating the healthcare system so that they can effectively advocate for themselves.
Continuity of Care: ECM staff works to ensure transitions between healthcare settings for clients through close collaboration with hospitals, rehabilitation centers, and other care providers.
